  1. Medical Record From UPMC Presbyterian August 18, 2002 which documents the 4MG Morphine injection and 25MG of Promethazine HCL, which left Dr. Berwick heavily sedated, barely conscious and defenseless for days.  View Document

  1. Letter from Robert P. Hussey, C.E.O. Emerald Owl Corporation, to Elizabeth Concordia December 28, 2002 which documents Dr. Berwick's formal request for the President and CEO of UPMC Health System to consider the facts concerning her inpatient stay at UPMC Montifiore.  View Document

  1. Letter from Elizabeth Concordia to Dr. Berwick January 13, 2003 which documents the fact that Dr. Solano committed the act of referring Dr. Berwick to the Physicians Health Program. He claims that the referral was for medical reasons and not related to any drug or alcohol problem. Secondly, that he was apologetic over the "miscommunication." The letter of course toned down the severity of the miscommunication by not mentioning the extreme nature of his accusations.  View Document

Hussey's attempt to obtain all pertinent documents related to Dr. Berwick's erroneous referral. Note that there are eight (8) specific and key questions asked in the letter from Dr. Berwick.   1. Letter form Dr. Thomas Hobbs, Physician's Health Program to Dr. Berwick December 2, 2002 which documents their response to Dr. Berwick's request for detailed information about her referral. The letter briefly states that Dr. Berwick was referred to the PHP following an episode of elevated blood pressure accompanied by symptoms of memory disruption. Note that Dr. Hobbs avoided responding to most of the questions asked in item 5 above (the request for information). View Document

  1. Letter from Dr. Berwick to Mr. Tim Finnerty, Attorney for Allegheny General Hospital, August 31,2005, requesting all documents pertaining to her termination at AGH for the purpose of credentialing at UPMC South Side and Passavant Hospitals.  View Document

  1. Letter form Dr. Cerul, PHP Psychiatrist, September 5, 2002. This letter contains lies and an addendum documenting Dr, Solano's uncomfortable feelings about Dr. Berwick's release from the PHP program.   View Document

  1. Letter from Dr. Berwick to Mr. McCormick, Investigator, the Pittsburgh branch of the Commonwealth of PA Department of State, Bureau of Enforcement and Investigation. This summary letter of what had been done to Dr. Berwick was initially sent as requested in January 2005. Following 6 months of so called investigation, Mr. McCormick was replaced by Investigator Mr. Monaghan and Dr. Berwick had to send this document again and start the process over. View Document

This Document is the identical letter sent to Mr. McCormick, which had to be sent again in June.
